Initial Intake Form & Consultation.
Once you reach out to our office, our provider will contact you with next steps, getting you on our schedule and sending you intake paperwork for you to fill out prior to your first consultation.

If needed, our provider will recommend necessary testing, and schedule a time to review your testing with you. She will provide in-depth insight to your functional testing and symptoms.

Individualized Treatment Protocol.
Our provider will offer unique lifestyle, nutritional, and supplemental recommendations to help you meet your health goals. You will receive a summary of these to your email.
FAQs
What is telemedicine?
Telemedicine allows healthcare providers to evaluate, diagnose, and treat patients remotely using HIPPA compliant video and audio platforms. You can be assured that your conversations with the provider and your medical records are kept secure and private.
What technology is required for a virtual visit?
You may use any computer, tablet, or smartphone that has web camera and video capability. Performance may be enhanced by connecting through Wi-Fi when possible.
Does Growing Whole accept insurance?
No, Growing Whole is a fee-for-service, cash-based practice that does not accept insurance. We keep our prices as low as possible in order to be affordable for our patients.
You will receive an invoice for your visit, which you may be able to submit to your insurance for an out-of-network claim. Most insurance plans cover telemedicine visits, but most do not cover functional medicine testing. If you intend to file a claim, we suggest that you check with your insurance provider to be sure.
We accept all major credit cards.
How will I pay for the services provided for me?
You will be charged at the time of your appointment. Our Electronic Medical Record is linked to the Square App and it takes all major credit cards. We accept HSA and FSA payments for visits, testing, and supplement payments. You will be issued an invoice for your records after your payment.
What if I need to cancel or miss my virtual appointment?
As a courtesy to other patients and the provider, please provide adequate notice if you need to cancel or change an appointment.
If you cancel within 12 hours of your appointment, your credit card will be charged 50% of the cost of the visit.
If you miss your appointment without any notification, your credit card will be charged for the full cost of your visit.
What are Growing Whole’s operating hours?
Growing Whole has varying hours. We strive to provide several blocks of convenient appointments times daily depending on our providers’ availability.

How quickly will it take for for us to hear back from Growing Whole?
Most days, you can expect to have a reply within 6 hours of reaching out. However, on the rare occasion it could take longer. If you contact us after 5pm, you can expect us to reply the next day.
What if I can’t get in contact with you right away and I am worried about my symptoms?
At Growing Health, we will do our best to be a helping hand in as many circumstances as possible. However, we do not want to completely replace the Urgent Care or Emergency Room. If you or your child are experiencing urgent health issues (trouble breathing, anaphylactic issues, severe dehydration, listlessness), please go to the Emergency Room.
What if my provider cannot diagnose my problem with a telemedicine visit?
In some cases, the provider may discover a more serious condition that requires you or your child to be seen in person for a more thorough evaluation. In these instances, the provider will work with you to determine the appropriate level of care and timing for follow-up (in person visit with Growing Whole provider, urgent care, or emergency room).
In every case, our goal is to make sure that you or your child receives the appropriate care for your condition.
